reconstituted vial left out for five hours — bin it or not
Took it out to draw, got distracted by a phone call, found it on the counter five hours later. Room temperature, maybe 21°C, out of direct light.
It is reconstituted, about two weeks into its in-use window, roughly four punctures. Looks completely normal, clear, no particulates.
Genuinely do not know whether this is a "fine, carry on" or a "bin it". Everything I can find online is about the powder.

Five hours at 21°C is fine. Not ideal, but fine, and I say that as the most paranoid person in this community.
The reason: in-use stability windows for preserved multi-dose solutions are not knife-edges. The 28-day refrigerated convention has enough margin in it that a single five-hour room-temperature excursion is a small fraction of the total exposure budget. It is not zero and it is not a disaster.
What would change my answer: if it had been 30°C, or if it were near the end of its window, or if it were reconstituted with sterile rather than bacteriostatic water. Which one applies to you?
bacteriostatic, about halfway through the window, and the room was cool. so all three of the things that would make it worse are absent.
Then carry on, note the date, and stop worrying about it. Honestly the bigger risk in your situation is that you now have a vial you are anxious about and anxiety makes people do odd things like starting again with a fresh vial they cannot afford.
the sterile-water case is the one where i would bin without hesitation, and it is the one people conflate with this. no preservative means no in-use window at all.
i genuinely did not know they were different until about a month ago and i have no idea how i avoided the mistake.
you avoided it because the person who sold it to you sent the right one. that is the whole mechanism and it is not reassuring.
Leaving this up as a reference question. The powder-versus-solution distinction is the most useful single idea in this community and this thread is a clean worked example of applying it.
the actual decision rule is visual plus time. cloudy or particulate after it has had time to dissolve is a discard and not a debate. clear and inside its window is fine.
also — asking is the right move and you did it before injecting rather than after. that is the correct order of operations and a lot of people get it backwards.
bin it, why risk it
because "bin it" as a default answer to any excursion means people bin material they cannot afford to replace, and then they take longer breaks than they meant to. risk is not one-sided.
