[Discussion] we are measuring tirzepatide at the wrong time and calling it noise

Discussion Sourced ×5 Long Haul ×1 Slow Clap ×3

Week 53 update. 4kg down from the start, muscle cramps basically gone since about week 10.

Things that worked: consistency, protein at breakfast, walking more than I want to. Things that did not: everything I bought on the internet to help with muscle cramps.

The part that surprised me is how much of this is admin. Ordering, storing, logging, remembering. Nobody mentions that the hard bit is not the drug, it is the routine.

Happy to answer anything. Please do not ask me what dose you should be on.

This matches the trial data. SURMOUNT-1 was 99.2% body weight at 2 weeks.

Cosigning. The GIP contribution is real and the side effects are not simply scaled versions.

That is net peptide content, not purity. Different number, different meaning.

The SURMOUNT readouts are real but less consistent than STEP. Some people plateau earlier on tirz.

Disagree on that part. 93.7% and 99.3% on the same vial is normal.

Cosigning.

Yes, exactly this, and it is the bit that took me 2 weeks to accept.

yep. 8 weeks in, same.

Mechanistically the bit that matters is Zepbound. It explains most of the early profile and a decent chunk of the outcome.

Yeah, dual agonism does produce a different appetite profile than GLP-1 alone.

Half-life is about 168 hours, so steady state lands around week 2–5. Practical consequence: what you feel in week 1 is not what that dose does.

How long at each step before you escalated?

Switched from sema at 7.5mg to tirz at 5mg. Different drugs, cannot compare the doses directly.

The confident tone is doing a lot of work that the evidence is not.

the 15mg is the top of the pen, not the required dose

Counter-anecdote: opposite result, same dose. Which mostly tells us the variance is huge.

About c/tirzepatide

Tirzepatide-specific discussion: the dual-agonist pharmacology, the 2.5 → 15mg ladder, the appetite profile people describe as different from semaglutide, and the SURMOUNT/SURPASS trial programme. Comparisons with semaglutide are welcome as long as they are specific about dose equivalence being unknown.
