Denied for prior auth, resubmitted with step therapy documentation, approved on appeal. The criteria exist, you just have to meet them.

External review overturned the denial. Took 17 weeks but the process exists.

Came off after 41 weeks for financial reasons. Regained about a third over the following year. Back on now at a much lower dose.
