Denied for prior auth, resubmitted with appeal documentation, approved on appeal. The criteria exist, you just have to meet them.

The single most useful thing I did was write the concentration on the vial. Stopped all the confusion after that.

Slight fix: the number was 96.8, not 97.9. Decimal point, but a fairly consequential one.
