someone explain 15mg to me like I have not read a paper in years

Titration Cold Box ×5 Sourced ×1

Something I noticed reading old threads that I have not seen said out loud.

The advice on SURPASS in this community changed substantially around the start of last year, and nobody went back and updated the older posts. So depending on which thread the search engine hands you, you get two contradictory answers with the same confidence.

I have listed what I think the current consensus is below. Correct me — that is the point of posting it.

Three years on this site and the questions have not changed at all. Which is depressing or reassuring depending on the day.

SURMOUNT data is messier than STEP data but the weight loss is there

This matches the trial data. SURMOUNT-1 was 94.2% body weight at 3 weeks.

dual agonism is Mounjaro, do not generalise from semaglutide

This matches the trial data. SURMOUNT-1 was 98.1% body weight at 15 weeks.

Week 63 on Mounjaro and 38kg down. The appetite reduction feels different from sema in ways I do not have words for yet.

the 15mg is the top of the pen, not the required dose

the muscle cramps from tirz is different, not necessarily worse

Disagree. What you are describing is consistent with SURMOUNT, not with what you concluded.

What was the trial arm you are quoting from? SURMOUNT-1, -2, -3 are all different.

do not crack the pen, we have been over this

the nausea from tirz is different, not necessarily worse

Tried both. Sema worked, tirz worked better for me, but my injection-site soreness profile was worse on tirz.

This is the "correlation is mechanism" thing again. You changed three variables at once.

the GIP side does different things than the GLP-1 side, that is the point

About c/tirzepatide

Tirzepatide-specific discussion: the dual-agonist pharmacology, the 2.5 → 15mg ladder, the appetite profile people describe as different from semaglutide, and the SURMOUNT/SURPASS trial programme. Comparisons with semaglutide are welcome as long as they are specific about dose equivalence being unknown.

  1. "Tirz is better than sema" needs a comparator dose or it is a vibe, not a claim.
  2. Vial-splitting and self-compounding discussion stays in c/reconstitution.
  3. No pen-cracking tutorials. Discussing that people do it is fine; instructions are not.
  4. Independent community. Nobody here sells anything, and anyone who tries is banned.
  5. Not medical advice. Describe what you did; never prescribe to a stranger.
  6. Claims need evidence. Batch numbers, dated screenshots, independent test reports, or a citation.
