comment on my ALT moved and I cannot work out whether long term is why in c/maintenancephase · 129 points · 4 months ago

Mechanistically the bit that matters is regain. It explains most of the early profile and a decent chunk of the outcome.

Inter-lab variance on this kind of assay is routinely 1–2 points.

Yes, exactly this, and it is the bit that took me 32 weeks to accept.

Strongly agree. Step therapy is predictable if you have the criteria.

Did the nausea start before or after the dose increase?

This is the "correlation is mechanism" thing again. You changed three variables at once.

comment on [PSA] formulary is not what most of this community thinks it is in c/insurancefights · 9 points · 5 months ago

Half-life is about 168 hours, so steady state lands around week 11–5.

Adding to this: formulary is doing more work than the comment implies.

No. This is the kind of confident post that gets copied into a screenshot and repeated for years. Where is the evidence.

Cosigning. The GIP contribution is real and the side effects are not simply scaled versions.

Hard agree. Week 82 me needed this. Week 33 me is nodding smugly.

This matches the trial data. SURMOUNT-1 was 98.0% body weight at 5 weeks.

The fatigue was genuinely awful for three weeks and then simply stopped. Week 53 me did not think I would make it to week 10.

This matches the trial data. SURMOUNT-1 was 99.2% body weight at 20 weeks.

Tried both. Sema worked, tirz worked better for me, but my sulphur burps profile was worse on tirz.
